A 35-year-old machine operator was killed in the Wildcat Hills Mine in Eldorado, when he got pinned between a coal hauler and the mine wall. 

The report shows the machine's operator had reduced visibility and could not see that the victim, Nathaniel Clarida, was there.

There were two other accidents at the same mine in 2011, but they didn't result in death. Investigators also blame reduced visibility for those incidents.
